Collegiate Team Requirements

• Team Captain: Main contact person of the team
• Team Composition: 5 main players + 1 optional substitute
• Coach / Manager (Optional): can be declared in the form
• All players of the team must belong to the same school.
• Only one Moonton account per player.
• MDL/MPL Players: To be announced
• Players must be part of the Philippine Collegiate Championship Discord.
• Parental consent required if the player is 17 years old or below.
